Creatinine Clearance & eGFR Calculator

Estimate creatinine clearance with the Cockcroft–Gault equation and eGFR with the race-free CKD-EPI 2021 equation, in mg/dL or µmol/L, with every step shown. For healthcare professionals.

Sex (as the equations use it)
years

For Cockcroft–Gault; eGFR doesn't use weight

For ideal and adjusted body weight

eGFR (CKD-EPI 2021)
69 mL/min/1.73 m²
CrCl, actual weight
74.1 mL/min
CrCl, ideal weight
65.2 mL/min
CrCl, adjusted weight
68.8 mL/min
  • CKD-EPI 2021: 142 × min(1.2 ÷ 0.9, 1)-0.302 × max(1.2 ÷ 0.9, 1)−1.200 × 0.993860 = 69.2, reported as 69.
  • Cockcroft–Gault: (140 − 60) × 80 kg ÷ (72 × 1.2) = 74.1 mL/min.
  • Ideal body weight (Devine) 70.5 kg; adjusted body weight 70.5 + 0.4 × (80 − 70.5) = 74.3 kg; BMI 26.1. Which weight to use for Cockcroft–Gault follows your local policy and the drug label.

How to use it

  1. Choose the sex the equations use, then enter age and serum creatinine (switch between mg/dL and µmol/L).
  2. Enter weight for Cockcroft–Gault, and height if you want the ideal and adjusted body weight versions.
  3. Read eGFR and the three creatinine clearance figures, and check the working underneath.

Formulas

CKD-EPI 2021 (creatinine, without race): eGFR = 142 × min(Scr/κ, 1)^α × max(Scr/κ, 1)^−1.200 × 0.9938^age × 1.012 [if female], with κ = 0.7 (female) or 0.9 (male) and α = −0.241 (female) or −0.302 (male). Scr is in mg/dL and the result in mL/min/1.73 m².

Cockcroft–Gault: CrCl = (140 − age) × weight (kg) ÷ (72 × Scr) × 0.85 [if female], in mL/min. Ideal body weight (Devine): 50 kg (men) or 45.5 kg (women) + 2.3 kg per inch over 5 feet. Adjusted body weight: IBW + 0.4 × (actual weight − IBW).

Worked example

A man of 60 with serum creatinine 1.2 mg/dL (106 µmol/L), 80 kg and 175 cm: CKD-EPI 2021 gives 142 × (1.2 ÷ 0.9)^−1.200 × 0.9938^60 = 69 mL/min/1.73 m². Cockcroft–Gault gives (140 − 60) × 80 ÷ (72 × 1.2) = 74.1 mL/min with actual weight. His ideal body weight is 70.5 kg and his adjusted weight 74.3 kg, giving 65.2 and 68.8 mL/min. For a woman with the same numbers the figures are 52 mL/min/1.73 m² and 63 mL/min.

KDIGO GFR categories (for reference)

CategoryGFR (mL/min/1.73 m²)Term
G190 or moreNormal or high
G260–89Mildly decreased
G3a45–59Mildly to moderately decreased
G3b30–44Moderately to severely decreased
G415–29Severely decreased
G5Under 15Kidney failure

KDIGO 2024 notes that without evidence of kidney damage, neither G1 nor G2 meets the criteria for chronic kidney disease; a category describes a number, not a diagnosis.

Frequently asked questions

What's the difference between creatinine clearance and eGFR?

Cockcroft–Gault estimates creatinine clearance in mL/min from age, weight, sex and serum creatinine, and isn't scaled to body size. CKD-EPI estimates glomerular filtration rate per 1.73 m² of body surface area and doesn't use weight; it's the equation laboratories report. Many older drug labels were written with Cockcroft–Gault, which is why both are still used.

Why is there no race factor?

The 2021 CKD-EPI equation was refitted without race (Inker and colleagues, NEJM 2021), and the National Kidney Foundation recommends it for adults. The 2009 version multiplied the result for Black patients; this calculator uses only the 2021 equation.

Which body weight should I use for Cockcroft–Gault?

The calculator shows all three. The FDA's 2024 guidance on renal impairment notes that ideal or adjusted body weight gives a more accurate estimate than actual weight in overweight or obese people, and a 2012 study (Winter and colleagues) found actual weight least biased when underweight, ideal weight at normal weight and adjusted weight when overweight. Follow your local policy and the drug label.

Can I enter creatinine in µmol/L?

Yes. Switch the unit and the calculator divides by 88.4 to get mg/dL before applying the equations; switching converts the value already typed.

When are these estimates less reliable?

Both rely on serum creatinine, so they assume kidney function is stable and muscle mass is typical for the person's age and sex. Rapidly changing creatinine, very high or low muscle mass and amputations make them less accurate, and both are for adults only.

Can I use the result to choose a drug dose?

No. This page shows the arithmetic of published equations for professionals to check. Dosing decisions belong to the prescriber, the drug's label and local guidelines.
